Change of Graduate Advisor Request

The Change of Advisor Request form (pdf) is required if a student would like to request a change of advisor.

Form must be completed and returned to the Graduate College. Approval from the Program Chair is required before the change can be made.

Confirmation of the change will appear on the student's goNSU account.
